Are stomach ulcer dangerous and its curable?

Is stomach ulcer dangerous?

In stomach ulcers “an excessive amount of acid is secreted”. With the proper medication and follow up, most of the peptic ulcer is curable. If you don’t take proper medication and precaution. Produce a dangerous effect in the stomach.

A stomach ulcer is dangerous if hemorrhage symptoms (heavily bleeding in the gut) produced. So, if you took a proper medication and avoid such unnecessary thing (smoking, over-drinking alcohol) don’t worry, it should be curable.

➤ Regulation of gastric acid secretion

stomach ulcer means damage to its mucosa and inner tissue to exposed a pepsin and acid is known as a stomach ulcer.

the secretion of the gastric acid in the stomach by the parietal cell. a parietal cell is regulating by the Ach, gastrin, histamine and prostaglandin E2.

Binding a histamine, gastrin, and Ach to its specific receptor. Increase the gastric acid secretion in the stomach.

Also, a binding a prostaglandin E2 to its receptor, decrease the gastric acid secretion. Such various phase of the stomach ulcer-hormonal, basal and cephalic.

Proton – pump H+-K+ ATPs are playing an important role in the gastric acid secretion.

➤ Is Peptic ulcer curable?

Most of the peptic ulcer is curable. But you don’t take proper medication and not follow a doctor advise it’s maybe dangerous for your health.

If a stomach ulcer is due to H. PYLORI infection. The prescriber can be given to such medication. in medication that kills infection (antibiotics) & reduces the gastric acid secretion (proton pump inhibitors).

But if your ulcer is not due to bacterial infection. The prescriber is given the proton pump inhibitor, acid blocker, and antacid drugs to reduce acid secretion.

You will be followed your prescriber's advice and take to medicine on time. Your stomach ulcer can be curable. And not a stomach ulcer is dangerous.

➤ Peptic ulcer healing time

Minor an ulcer is healing itself or you can take some medicine like antacid or H2 blocker.

But in severe condition, peptic ulcer healing time is like 4 to 12 hours if you can take a proton pump inhibitor.

Simply H2 blockers are work within a 60 min to suppress the acid but effect produce within 4 to 8 hours. Some antibiotics treatment like (triple and quadruple therapy) can take at least 2 weeks.

➤ Will stomach ulcer heal itself?

some minor stomach ulcers produced due to the imbalance of the acid and alkaline in the stomach. This minor ulcer is healed itself. But, during these stages taking some precautions. But in severe conditions, take proper medication and precaution are required.

➤ Peptic ulcers are caused by various factors:

➜ Most common causes are the helicobacter pylori, that cause stomach inflammation and stomach infection
➜ Use of the NSAIDs drugs in frequents
➜ Overdrinking an alcohol
➜ Due to a radiation therapy
➜ Due to tobacco consumption (smoking)

➤ What is peptic ulcer symptoms:

➜ Burning to abdominal pain
➜ Indigestion of food
➜ Sometimes chest pain
➜ Dark stool
➜ Nausea and vomiting

➤ Drugs for peptic ulcer:

➣ proton pump inhibitor:

➩ Omeprazole (Prilosec)

➩ Pantoprazole (protonix)

➩ Esomeprazole (Nexium)

➩ lansoprazole (prevacid)

➩ Rabeprazole (pariet)

if you can a take a timely a proper medication. don’t stomach ulcers are dangerous. The most powerful medication in stomach ulcer condition is proton pump inhibitors. In today's market various proton pump inhibitors are available.

this all proton pump inhibitors play an important role in decrease gastric acid secretion. These agents are the most powerful inhibitors of gastric acid secretion. Both time of secretion fasting and stimulated are inhibited.

It is administered to 30 min before a meal because food can stimulate the secretion of the acid. And its decrease the absorption of the proton pump inhibitors.

This agent’s half-life around 1.5 hours. So, suppress the stomach ulcer within 24 hours. Available in powder and enteric-coated formed.

Side effect of proton pump inhibitors

Short term side effect

➜ Skin rashes

➜ Arthralgia

➜ Abdominal pain

➜ Headache

➜ Nausea and vomiting

Long term side effect

➜ Decrease vitamin B12 infection

➜ Pneumonia infection increase

➜ Osteoporosis

➜ Gynecomastia

➜ Hypergastrinemia

➣ H2 – blocker:

➩ Cimetidine (Tagamet)

➩ Ranitidine (Zantac) (this drugs withdrawal now)

➩ Famotidine (Pepcid)

➩ Nizatidine (Tazac, Axil)

These all drugs are competitively inhibiting the H2 receptor on the parietal cell and block the gastric acid production. It decreases gastric acid secretion by gastrin, Ach, and foods.

H2 blockers are less potent than proton pump inhibitors. Its administered through orally, and well it is well absorbed. Metabolize by the liver and excreted through urine.

It's not suitable for the people who have a kidney problem and a pregnant lady.

Side effect of H2 blocker:

Not have any side effect produces but, some minor side effect produces in some patients.

➜ Diarrhea

➜ Headache, dizziness

➜ Skin rashes

➣ Antacid:

Antacid are “anti-acid”. It is not suppressed acid production. But it increases the gastric PH in the stomach and also an increase in the alkaline environment in the stomach.

Antacid should be capable of the neutralized gastric acid in the stomach.

Mainly two type of antacid in the market:

Systemic antacid:

➩ Sodium bicarbonate (Alka-Seltzer original, bromo seltzer)

Non-systemic antacid:

➩ Mg trisilicate (Gaviscon -2)

➩ Mg hydroxide (milk of magnesia)

➩ Ca carbonate

➩ Al hydroxide (Alterna GEL, Nephrox)

Systemic the antacid is neutralizing the gastric acid. But the duration of action of the systemic antacid is very short.

Sodium bicarbonate is used in the acidosis and treats the alkaline urine. Also, it should be avoided the congestive cardiac failure and hypertension patients.

Aluminum salt can cause constipation. Also, a magnesium slat can cause the diarrhoea.

Mg hydroxide is the rapid onset of the action. But Al hydroxide is acted a slowly but combination of both are producing a rapid and sustained-release effect.

It should be available as the suspension, powder and tablet formed. The tablet should be the chewed and swallowed for its better effect.

➣ Antibiotics:

Most of the antibiotics can kill the H. pylori infection. H. pylori can cause mucosal damage and inflammation. Also, these antibiotics are killing the H. pylori bacteria.

The combination therapy is the most common therapy for H. pylori infection.

Lansoprazole 30 mg BD +

Amoxicillin 1 g BD +

Clarithromycin 500 mg BD

Tetracycline 500 mg QID +

Omeprazole 20 mg BD +

Metronidazole 400 mg TID +

CSB 120 mg QID

Why triple therapy most popular for H. pylori infection?

➜ To promote rapid ulcer healing.

➜ To also prevent relapse.

➜ To also a eradicate the H. pylori infection.

➜ Delay the development of the resistance organism.

Some antimicrobial is used in the H. pylori infection.

➩ Amoxicillin (Moxatag, Amoxil and Larotid)

➩ Tetracycline (Sumycin, Actisite)

➩ Metronidazole (Flagyl)

➩ Clarithromycin (Biaxin)

But some drugs rapidly developed its resistance. Amoxicillin is avoiding the patients who have a penicillin allergy.

After giving a triple and quadruple therapy (14 days), to given proton pump inhibitors should be 6 more weak to enhance ulcer healing.
